If you are a new patient to our office, please take advantage of our new patient forms that can be filled out, printed and brought with you to your appointment. Bringing them with you will allow us to attend to your dental needs more quickly than completing them upon your arrival. Should you have had a Full Mouth Set of Xrays or a Panoramic Xray within the past 3 years, you may want to obtain a copy, as some insurance companies have exclusions and limitations as to how often they cover them.
